Here is an old prompt I wrote down in my notebook ages ago.
Since then I have played around with the concept (I have a title!) and even started an outline (which is as far as I will go since I don't write) and a couple of drawings for the 'verse ;)

Brothers in Name - Brian O'Connor, aka Brian Winchester, nee Campbell is raised as Sam and Deans older brother.
The premise is the following: In 1973 Samuel and Deana Campbell are killed by the YED, leaving 19-year-old Mary and 4-month-old Brian as orphans. Mary and John take Brian in and raise him as their own, basically getting a jump start on that family they thought was years down the line.
AN: One of the things that always bothered me in the SPN pilot is the implausibility of a four-year-old carrying a six-month-old down a full set of stairs and out the door. Dean as portraid by the kid actor is about the same size as my nephew and he wasn't allowed to hold his younger brother by about 4 years without supervision, let alone carry him. Children of that age just don't have that upper body strength, let alone the coordination to pull something like carrying a baby that size while running. So, in the above scenario - Brian is the one carrying baby Sammy out while dragging Dean along, also conveniently blocking Dean's view into the nursery (although in the pilot he was always in the hallway ;)
